1885723 - Clean up more jsm references

Following on from bug 1884623, the production code is a bit clearer with respect to jsm references, although there's some more to tidy up here, and we can make a good start on the test code.

Once this pass is done, we'll probably be in a good enough place that we can leave further clean ups until we start removing support for the old module loaders.

Note that I'm explicitly not touching searchfox urls with revisions in them unless it is in an area I know - I will post to the mailing lists that the teams themselves need to check and update them, as they're likely pointing to specific pieces of code.
